Free Online "Diabetes Self-Management Program" Offered by CCACC Health Center

CCACC Health Center will launch a six-week free online Diabetes Self-Management Program beginning Thursday, April 23. Designed for individuals living with diabetes and their family members, this program aims to provide comprehensive diabetes education and practical self-management skills to help participants better control blood sugar levels, prevent complications, and improve overall quality of life.

As modern lifestyles continue to evolve, diabetes has become a global public health concern, with an increasing trend toward younger onset. High-calorie and high-sugar diets, prolonged sedentary habits, and lack of physical activity all contribute to a higher risk of developing diabetes. As a persistent chronic condition, diabetes can lead to serious complications over time if not properly managed, including cardiovascular disease, kidney damage, vision problems, and skin infections.

The program will be led by Dr. Ling Chin (MD) and Ms. Kate Lu (LCSW-C), Director of CCACC Health Center. They will provide participants with essential knowledge about diabetes and guidance on developing healthy meal plans and appropriate exercise routines to build strength and endurance. Participants will also learn how to work more effectively with clinics and healthcare providers, and how to establish sustainable long-term healthy lifestyle habits to help manage symptoms. Interactive discussions will offer opportunities to share practical experiences and receive concrete daily management strategies, empowering participants and their families to face the challenges of diabetes with greater confidence.

Given the potential risks and complications associated with diabetes, early prevention and effective self-management are essential. Through healthy eating, regular physical activity, weight management, and maintaining mental well-being, individuals can significantly reduce the risks associated with diabetes. Whether you have recently been diagnosed or have been managing diabetes for years, CCACC Health Center warmly invites you to join this free online Diabetes Self-Management Program and make it a strong partner on your journey toward better health.

This workshop and related activities are funded by the Asian American Health Initiative (AAHI), Office of Community Affairs, Montgomery County Department of Health and Human Services.
