The CCACC Health Center is currently seeking qualified candidates for the following positions: Physical Therapist, Nurse Practitioner, and Licensed Master Social Worker (LMSW). Physical Therapist

Responsibilities:

· Evaluate patients' physical conditions through assessments, medical histories, and screenings.

· Develop and implement individualized treatment plans aimed at improving mobility, managing pain, and restoring function.

· Provide education on exercises and techniques for patients to continue their care at home, emphasizing affordable and accessible options.

· Work with patients who may have limited resources, adapting treatments to ensure effectiveness within these constraints.

· Collaborate with other healthcare providers, including physicians, social workers, and community health workers, to coordinate comprehensive care.

· Document patient progress, treatment plans, and outcomes accurately in the electronic medical record (EMR) system.

· Participate in community outreach activities, such as educational workshops or local health fairs, to promote physical health and wellness.

· Stay informed about community resources, public health programs, and support services available to patients in lower economic areas.

· Advocate for patient access to necessary services, assistive devices, or referrals to additional resources as needed.

 Qualifications:

· Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program.

· Current state licensure as a Physical Therapist in Maryland.

· Must have a minimum of 2 years' experience as a Licensed Physical Therapist.

· 1-2 years of experience in a community health, outpatient, or low-resource setting preferred.

· Familiarity with common conditions affecting underserved populations, such as chronic pain, mobility limitations, and post-surgical recovery.

· Strong communication and interpersonal skills, with an emphasis on empathy and cultural sensitivity.

· Ability to adapt treatments to the needs and circumstances of patients with limited resources.

· Proficiency in electronic medical record (EMR) systems.

· Bilingual skills (preferred but not required based on patient demographics).

Nurse Practitioner

Responsibilities:

· Conduct comprehensive patient assessments, including medical histories, physical exams, and screenings.

· Diagnose and manage both acute and chronic health conditions, including hypertension, diabetes, and respiratory illnesses.

· Develop and implement treatment plans with a focus on accessible and affordable care options.

· Prescribe medications and therapies as appropriate and within the scope of licensure.

· Educate patients on managing chronic illnesses, preventive health, and wellness strategies.

· Refer patients to specialists and social services as needed for additional support and resources.

· Collaborate with physicians, nurses, social workers, and other healthcare professionals to provide integrated care.

· Maintain accurate and timely documentation in the electronic medical record (EMR) system.

· Participate in community outreach, including health fairs, educational workshops, and local partnerships to increase access to care.

· Stay current on public health issues, community resources, and programs available to lower-income patients.

 Qualifications:

· Must have a minimum of 2 years' experience as a Nurse Practitioner.

· Master's degree in Nursing with a focus on Family, Adult, or Community Health.

· Current Nurse Practitioner (NP) license in Maryland and national certification (e.g., FNP, ANP).

· 2+ years of experience in a primary care or community health setting is preferred, especially with underserved populations.

· Knowledge of healthcare disparities and social determinants of health affecting lower economic areas.

· Strong communication skills, with a focus on empathetic and culturally sensitive patient care.

· Ability to work independently and as part of a multidisciplinary team.

· Proficiency in electronic medical record (EMR) systems.

· Bilingual skills (preferred but not required based on patient demographics).

Licensed Master Social Worker (LMSW)

Responsibilities:

· Conduct psychosocial assessments to identify client needs and develop individualized care plans.

· Provide counseling and supportive services to clients facing issues such as poverty, homelessness, family conflict, substance abuse, and mental health challenges.

· Connect clients with community resources, including housing assistance, food services, employment programs, healthcare, and other support systems.

· Assist clients in crisis situations by providing immediate support and intervention, while connecting them with appropriate services.

· Work with healthcare providers, social service agencies, and community organizations to coordinate comprehensive, wraparound support for clients.

· Maintain accurate documentation of client interactions, assessments, and care plans in the electronic medical record (EMR) system.

· Educate clients on coping strategies, community resources, and strategies for managing personal challenges.

· Advocate for clients within social service and healthcare systems to ensure they receive the support and services they need.

· Participate in community outreach and awareness programs to promote health, wellness, and access to social services in the area.

· Stay informed about public health trends, community resources, and social issues affecting low-income populations.

 Qualifications:

· Master's degree in Social Work (MSW) from an accredited institution.

· Active LMSW license in Maryland.

· Must have a minimum of 2 years of experience in social work or case management, ideally with underserved populations or in a community health setting.

· Understanding of social determinants of health and the unique needs of individuals in lower economic areas.

· Strong interpersonal, communication, and problem-solving skills, with an emphasis on empathy and cultural competence.

· Ability to work collaboratively within a multidisciplinary team.

· Proficiency in electronic medical record (EMR) systems.

· Bilingual skills (preferred but not required based on patient demographics).
