6.1.2020 CCACC Meal Delivery Program & Application Form
Chinese Culture and Community Service Center (CCACC) has received funding from Montgomery County’s COVID-19 Emergency Food Assistance Grant. From now until the end of August, we will provide the free lunch delivery service for seniors living alone in the community.
Qualifications:
1. Residents living in Montgomery County; and,
2. Over 60 years old, or under 60 years old but has physical or mental disabilities and has difficulty in obtaining food during the pandemic; and,
3. Live alone or only with spouse and have no other family members that can assist in obtaining food; and,
4. Live in the following apartments:
a. 95 Dawson Avenue Rockville 20850
b. 101 Odendhal Ave, Gaithersburg. MD 20878
c. 10250 Westlake Drive, Bethesda, MD 20817
d. 199 Rollins Ave, Rockville 20852
e. 4521 East West Hwy, Bethesda 20814
If you do not live in the above apartments but are still interested in participating in our meal delivery service, you can also register for our program and we will charge the cost of $20 per meal.
Application:
1. Call CCACC’s COVID‐19 Rescue Advisory Hotline 301‐663‐0983, or,
2. Referred by members of CCACC Adult Day Healthcare Center, CCACC Home Care, or CCACC Pan Asian Volunteer Health Clinic
Documents:
1. Residence Certificate in Montgomery County
2. Signed Application Form
Services:
1. Lunch box delivery three days a week (Monday & Wednesday & Saturday or Tuesday & Friday & Sunday)
2. Free related welfare resources consultation
The spots are limited for the service, please apply as soon as possible.
Unqualified:
1. Members of other day care centers: Most day care centers provide food delivery service and daily phone-call service to their members per the government’s request. If there is a food shortage problem, they will know it immediately and provide customized service accordingly.
2. People who live in a nursing home or group home
